Family Medicine Licensed Nurse - CCM

Position Summary

This position requires a driven, self-motivated candidate with a friendly and professional telephone presence and a strong organizational ability. Medical office background and medical terminology is a must.

Licensed Vocational Nurse

The essential functions include, but are not limited to the following:

• Talking to multiple health facilities in order to track patients
• Training in Chronic Care Management
• Organizing patients in spreadsheets
• Chart Auditing using a rubric
• Direct patient calls to schedule patients
• Time tracking
• Documenting history; recording and reporting pertinent observations and patient reactions to staff, as appropriate
• Documenting lab procedures and ensuring follow up on results by keeping records
• Using screening procedures, including medical history review, and observing and documenting patient vital signs as required
• Verifying equipment function through routine equipment maintenance and advanced troubleshooting; calibrating equipment utilizing approved testing procedures; monitoring quality control measures and protocols
• Training other technicians or other workers
• Assisting with or instituting emergency measures as needed for a crisis situation
• Performing other duties as assigned
