This is an exceptional opportunity for a Nurse Practitioner or Physician Assistant to join a busy, well-established outpatient rural primary care practice with a strong referral base and the full support of a robust health system network, conveniently located on the campus of Ascension Calumet Hospital.
- Join an established team of 1 physician and 5 APP's
- Call structure: 1:7
- Practice in a high-volume rural primary care clinic with a supportive, team-oriented environment within a busy and collaborative setting with an average of 12-15 patients per day
- On campus with Calumet Hospital which was recognized as a 2024 Top 100 Critical Access Hospital by the Chartis Center for Rural Health for outstanding performance among the nation’s rural hospitals.
- Provide comprehensive, patient-centered care by conducting health assessments, obtaining patient histories, ordering and interpreting diagnostic tests, diagnosing and treating acute and chronic conditions, and delivering preventive care services.
- Educate and counsel patients and their families on preventive health, treatment options, and community resources while collaborating with physicians to develop, implement, and document individualized care plans.

Licensure / Certification / Registration:
- Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
Education:
- Master's degree of Nursing required.
