- Thrive in a collaborative, team-based model of care
- Enjoy complex clinical presentations and developing treatment plans that fit patients' real-life circumstances
- Adapt readily to changing patient needs in a fast-paced, high-acuity primary care setting
- Are confident practicing at the top of your license
Responsibilities:
- Provide comprehensive primary care to patients with an emphasis on chronic disease management and preventative care
- Perform physical examinations, diagnose and treat acute and chronic conditions, and develop individualized treatment plans
- Prescribe and manage Medication-Assisted Treatment (MAT) for patients with opioid and other substance use disorders, in partnership with our behavioral health team
- Manage patients with co-occurring medical, mental health, and substance use needs
- Order, interpret, and communicate laboratory and diagnostic test results
- Deliver preventive care, including screenings, immunizations, and patient education
- Coordinate care with specialists, clinical pharmacists, mental health therapists, case managers, and community resources to address both clinical and social needs
- Participate in quality improvement initiatives and support organizational performance measures
- Promote a welcoming, respectful, and culturally responsive environment for all patients
