At Health by Design, we’re redefining primary care through a value-based care model—one that prioritizes outcomes over volume, relationships over transactions, and prevention over reaction. Our clinics are designed to give providers the time, tools, and support needed to deliver truly meaningful care.
We don’t measure success by RVUs—we measure it by healthier patients, stronger relationships, and sustainable care. Through longer visits, smaller patient panels, and an integrated care team, you’ll be empowered to focus on whole-person health, lifestyle medicine, and long-term outcomes.
