Family Support Coordinator for Indigenous Communities

This position serves as an important point contact for families, community partners, and Tribal citizens. Through coordination and compassionate service, the Family Support Coordinator supports the Family Support Center's commitment to strengthening stability for families of children with developmental disabilities and/or autism by protecting community trust and meeting the various needs of Tribal families and their households.
Service Navigation is the provision of comprehensive and individualized information, guidance, and support to families to address their needs, connect them to potential resources, assist with problem solving, and help navigate the service system. These services enable families to stay together in a nurturing environment, maintain relationships and be welcomed, contributing members in their communities.
Key Responsibilities:
- Assist families in caring for their family member(s) with IDD and/or ASD at home by providing individualized guidance, support and problem solving assistance, as well as help families access and coordinate potential community resources and services
- Must have knowledge of Public Housing, SNAP, SSI, Masshealth and other local, state and federal resources to better assist families
- Participate in Transition Planning, IEPs, ISPs, Whole Life Planning, Team Meetings
- Promote self determination values and self directed options
- Enhance the capacity of communities to value and support people with disabilities and their families
- Respond to the specific ethnic, cultural and linguistic needs of families
- Maximize community resources
- Completed all agency required training.
- Other duties as assigned
Working Conditions
- Physical activities include but not limited to climbing, balancing, stooping, kneeling, pulling, etc.
- Exerting in excess of 100 lbs. of force occasionally.
- Subject to both indoor and outdoor environmental conditions.
- The worker is subject to hazards such as moving parts, moving vehicles, etc.
