In this role, you will manage a diverse caseload of young people, providing vital support to them and their families throughout their transition period. You will act as a key bridge between services, ensuring no young person falls through the gaps.

Key Responsibilities:

  • Participate in and receive support to attend court hearings when required.

What We Are Looking For (About You):

  • Experience working with young people with disabilities, individuals with SEND or Care Leavers.
  • Knowledge of both Children’s legislation and Adult social care legislation, such as the Care Act 2014.
  • Strong communication skills and the ability to build trust with families during times of change.
  • Excellent report writing skills for assessments and support plans.
  • A relevant qualification in health, social care, youth work, or equivalent experience.

  • A healthy work-life balance with regular flexible working, including 2 days working from home.
  • A supportive team environment with regular supervision and excellent training opportunities.
  • Support and mentoring for specialized tasks like attending court hearings.
