Job Title: Paid Family Caregiver
Job Class: Health Care Provider
Job Description: Family member providing homecare services to a child under the direction of a Registered or Licensed Practical Nurse. Family member means a “child, parent, parent-in-law, sibling, grandparent, grandchild, spouse, or any other individual related by blood, and any other individual with a close association that is the equivalent of a family relationship”.
Immediate Supervisor: DON
Duties and Responsibilities:

• Completes all identified training within 14 days of hire.
• Provides patient with supports as per the treatment plan. Does not participate in activities identified as prohibited for the Paid Family Giver program.
• Completes documentation of service provision in AlayaCare.
• Reports inappropriate actions or conduct by any individuals in the home to the SCP Nurse Case Manager.
