Youth Haven Services is excited to announce the opening of our new Day Treatment Program located on the campus of New Vision School in Madison, NC. This new program represents another opportunity to fulfill our mission of supporting the healing of children, adolescents, and their families through empowerment and hope.

We are seeking a compassionate, organized, and self-motivated Case Manager to join our team as we launch this exciting new program. Position Summary

The Case Manager is responsible for providing comprehensive case management services for clients enrolled in the Day Treatment Program. This includes coordinating authorizations and reauthorizations for services, facilitating Child and Family Team (CFT) meetings, developing Person-Centered Plans (PCPs), submitting Service Authorization Requests (SARs), maintaining accurate documentation, and collaborating with families, school personnel, and community partners to ensure quality care.

Qualifications

Education

  • Bachelor's degree in a Human Services field is required.

Experience

  • Minimum of two (2) years of documented experience working with children and adolescents in a behavioral health, mental health, or human services setting.

Knowledge, Skills & Abilities

The ideal candidate will possess:

  • Knowledge of community resources and service systems.

  • Excellent written and verbal communication skills.

  • Strong organizational and time management abilities.

  • Experience using electronic documentation systems and Microsoft Office applications.

  • Ability to prioritize tasks, manage multiple responsibilities, and work in a fast-paced environment.

  • Strong critical thinking and sound decision-making skills.

  • Ability to build productive relationships with youth, families, schools, and community partners.

  • Initiative, flexibility, and a collaborative approach to problem-solving.

Travel

Services are primarily delivered within the school setting.

Essential Responsibilities

  • Coordinate all case management activities related to service authorizations and reauthorizations, including CFT meetings, PCP development, and SAR submissions. (Approximately 50% of duties.)

  • Collaborate with school personnel, social services professionals, clients, and families to develop and maintain Person-Centered Plans while ensuring client records and documentation meet agency standards. (25%)

  • Complete all required documentation accurately and within established timelines. (10%)

  • Attend Child and Family Team meetings and collaborate with Site Managers regarding client progress and treatment planning. (10%)

  • Perform other duties as assigned to support the success of the Day Treatment Program. (5%)
