Head of Engineering
Overview
Compiled Health is a clinical AI research company. We are building a future where the standard of care is executable: where what medicine knows is what medicine does, at every visit, in every setting. The accumulated, evidence-graded knowledge of medicine still lives in documents built to be read, not executed. We compile that knowledge into decision artifacts that are measurable against the standard, deterministic where they must be, and traceable to their source evidence. Our compiler is live in clinical production today: clinicians in public health networks act on its recommendations every day.
About the Role
A compiled artifact only matters if it reaches the right clinician, with the right patient context, through a system whose behavior can be reconstructed later. Engineering owns that path: the clinician-facing product, artifact execution and delivery, interoperability, deployment infrastructure, security, and reliability.
You will lead this work and remain one of its principal builders. You will make architectural decisions in code, resolve difficult production failures, turn early clinical integrations into a durable platform, and hire the team that carries it forward. You will report directly to the CEO and work closely with the members of technical staff building the compiler, the clinicians validating its artifacts, and the health-system partners deploying them.
What You'll Do
Own the production architecture from compiled artifact to point-of-care use.
Define the interfaces between the compiler and runtime, preserving provenance, versioning, deterministic behavior, auditability, and safe rollback.
Ship across the stack; review critical designs and code; lead the diagnosis of difficult production and integration failures.
Turn each clinical deployment into reusable platform capabilities without erasing meaningful differences among jurisdictions and workflows.
Establish the engineering bar for testing, observability, security, releases, and incident response; hire and develop a small, exceptional team.
Skills and Qualifications
Minimum qualifications:
A record of building and operating a consequential production system end to end.
10+ years of professional software engineering experience, including substantial experience hiring and leading engineers while remaining an active technical contributor.
Breadth across backend, data, infrastructure, and product engineering, with unusual depth in at least one.
Strong judgment around state, versioning, provenance, observability, security, and failure recovery.
Product judgment: you can make complex behavior legible without concealing the details on which a clinical decision depends.
Clear, precise communication across engineering, research, clinical, and external teams.
Bonus qualifications:
Experience with FHIR, HL7, EHRs, or other complex interoperability environments.
Experience in healthcare, regulated software, or another safety-critical domain.
Experience with compilers, rules engines, developer platforms, or rigorously evaluated ML systems.
Logistics
Location: This role is remote, with offsites multiple times per year.
Work authorization: You must be eligible to work in the United States or Canada. We are not able to sponsor visas at this time.
Benefits: Compiled Health offers health, dental, and vision benefits, flexible PTO, and paid parental leave.
We are an equal opportunity employer and evaluate candidates on merit alone.
