Health Coach (Fixed Term, Part Time, 32 hours)
Locations: Local Doctors Ranui and White Cross New Lynn
Local Doctors and White Cross are part of Tāmaki Health, working side by side to deliver accessible, community based care across Aotearoa. Local Doctors focuses on ongoing primary care within the community, while White Cross provides responsive urgent and general practice support when it matters most. Together, these services ensure patients can access the right care, at the right time, in a way that is connected and continuous.
About the role
This is a clinic-based role working as part of a general practice team. You will support patients to better understand and manage their health through short, structured coaching conversations. The focus is on building confidence, strengthening self-management, and helping patients take practical steps to improve their wellbeing.
You will work alongside GPs, nurses, and Health Improvement Practitioners, contributing to coordinated care. The work is varied, from one-on-one coaching in appointments to supporting group programmes and connecting patients with wider services when needed.
What you will do
- Deliver brief, structured health coaching as part of routine primary care
- Support patients and whānau to manage long term health conditions
- Work alongside clinical teams to ensure cohesive and coordinated care
- Maintain accurate and timely documentation
- Contribute to group based education or wellbeing programmes
- Build strong working relationships within and across clinic teams
What you must have
- Lived experience of managing a health condition, personally or as a caregiver
- Experience working with people in a health, wellbeing, or community setting
- Strong communication skills and the ability to build trust with a wide range of patients
- Confidence using digital systems and maintaining clear records
- Ability to work within structured, evidence-based approaches
- At least a secondary school qualification and the right to work in New Zealand
Nice to have
- Experience in primary care, urgent care, or community health
- Group facilitation or programme delivery experience
- Additional languages relevant to the communities we serve
- Experience working with Māori, Pacific, or diverse populations
- A health related qualification or relevant training
About the team
You will be part of the Wellness Support Team, a group of practitioners working within clinics to provide coordinated support for patients and whānau across a range of wellbeing needs.
The team brings together different roles to support practical, day to day care and improve how patients access and experience services.
