Health Coach - Heart Wellness 1.0 FTE Evening
Health Coach - Heart Wellness at Providence Mother Joseph Plaza Portland. This is a Full-Time (1.0 FTE), Evening Shift position, 40-Hour Work Week, Monday to Friday, from 10AM-7PM.
This position provides health coaching, under the Basecamp Prevention and Wellness suite of programmatic offerings, with an emphasis on behavioral change modification in the areas of movement, nutrition and wellness. The Health Coach functions independently within community partner settings, onsite programmatic settings and within care clinics. The Health Coach is responsible for developing relationships with participants/patients through a combination of communication and methods aimed at increasing awareness, enhancing motivation, changing behavior and creating an environment that achieves optimal health and quality of life. Health Coaches promote Basecamp Prevention and Wellness tools and services to support participants health that align with wellness industry best practices. Coaches work collaboratively with participants and patients to develop and coordinate services that promote sustainable lifestyle change.

Required Qualifications:
- Bachelor's Degree Documented accredited university course work in a related health field such as exercise science, community health or health promotion or equivalent educ/experience
- 3 years Working experience as a health coach.
- 1 year Providing MNT and individual counseling.
- 1 year Teaching and modeling self-management skills, concepts, resources, and tools related to health, lifestyle change and behavior modification.
- 1 year Lifestyle Medicine, Health Coach Board Certification (NBHWC).
