Health Home Care Manager - Tri County (5154)
Basic Function/Overall Responsibilities: The Health Home Care Manager works in collaboration and continuous partnership with youth and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources. The Care Manager focuses on developing, monitoring, and evaluating interdisciplinary care with the goal of stabilizing and enhancing the enrolled child's health and well-being.
Principal Assigned Responsibilities:
- Responsible for eligibility verification of youth referred for Health Home Care Coordination services.
- Conduct initial and ongoing assessment of the enrolled youth and his/her family's strengths and needs using the Child and Adolescent Needs and Strengths (CANS) or similar tool.
- Create and promote adherence to an interdisciplinary care plan to include crisis and emergency response planning, developed in coordination with the youth, his/her family/caregiver(s), service providers, and the youth's social supports.
- Monitor progress and evaluate effectiveness of care plans facilitating changes as needed.
- Serve as the contact point, advocate, and informational resource for enrolled youth, his/her family/caregiver(s), and the youth's care team.
- Develop positive reciprocal working relationships with other service providers/agencies through timely communication, inquiry, and follow-up regarding transitions in care and treatment recommendations.
- Provide education and facilitate linkages to youth and family/caregiver(s) about relevant community
- resources.
- Complete all required training as established by Department of Health, Lead Health Home(s), and agency within prescribed timeframes.
- Ensure compliance with agency and program policy and procedures.
- Participate in performance improvement activities as defined by the Quality Improvement Program.
- Participate in on-call rotation as necessary.
- Other associated tasks as assigned by Supervisor.
