About the role
The Wellness Support Team is expanding its after-hours service and is seeking an endorsed Health Improvement Practitioner to join our award-winning team.
This role is based within an Urgent Care setting, providing timely mental health, addiction, and wellbeing support when many other services are unavailable. Working alongside GPs, nurses, and Health Coaches, you will deliver brief, evidence-based interventions that help people address challenges early and build practical strategies for positive change.
This is a 0.6 FTE position, with days and hours negotiable. The role includes evening and weekend work.
What you will do
As a core member of the clinical team, you will provide brief behavioural health interventions and support people experiencing a range of mental, physical, and social wellbeing concerns.
You will work collaboratively with clinic teams to provide integrated care, contribute clinical expertise, support patients to access additional services when required, and maintain clear and accurate clinical records. The role uses the Health Improvement Practitioner model and Focused Acceptance and Commitment Therapy, delivering practical support through brief consultations embedded within primary care.
What you will bring
To be considered for this role, you must be an endorsed Health Improvement Practitioner with experience working in New Zealand primary care.
You will also have:
- Registration with an appropriate professional body and a current Annual Practising Certificate, or full NZAC membership.
- Experience delivering brief, evidence-based interventions.
- Strong communication and relationship-building skills.
- Confidence working within multidisciplinary teams.
- A commitment to culturally safe practice and equitable health outcomes.
- The ability to engage effectively with diverse communities across Tāmaki Makaurau.
About the Wellness Support Team
Established in 2012, the Wellness Support Team provides integrated mental health and wellbeing support within primary care settings across Auckland. The team brings together Health Improvement Practitioners, Health Coaches, community support workers, and clinical leaders who share a commitment to accessible, person-centred care.
The programme has been recognised nationally, receiving the award for Best Mental Health Programme at the New Zealand Primary Healthcare Awards.
