Health Services Coordinator (HSC)/Licensed Practical Nurse (LPN)

Schedule: Full-Time. Requires some direct care. Weekends and holidays as needed. Participates in “On-Call” responsibilities.

What you will bring to our team:

The right person for this position will be a currently licensed LPN and possess a participatory leadership style and ability to work in multiple services with minimal supervision. They will enjoy problem solving and have a passion for ensuring quality services. They must have the ability to acquire an understanding of all applicable Department of Human Services and/or Department of Health rules and regulations and company policies.

• Current LPN license
• Knowledge in the field of comprehensive homecare, day services, and 245D non-residential services.
• 1 year of experience providing direct care to individuals with disabilities or related conditions
• Valid driver’s license and pass a Motor Vehicle Record check.
• Must not be disqualified by criminal background studies.
• Meet the physical requirements as outlined in the physical assessment.
• Possess good oral and written communication skills and organizational skills.
• Able to read and write in English.
• Basic knowledge of Microsoft Office and Internet Browsers
