🕘 Working Hours: 40 hours per week
📅 5-day work week, including weekends & Public Holidays
Shift timings:
• 8.00am – 5.00pm
• 8.00am – 5.30pm
• 8.00am – 6.00pm
• 12.30pm – 8.30pm
• May extend to 10.00pm in the future
🌟 What You'll Be Doing
Join a growing hospital-at-home programme supporting patients who receive care outside the traditional hospital setting.
As a Program Coordinator, you will play an important role in coordinating daily operations, patient intake and discharge, logistics and communication between different healthcare stakeholders.
Key Responsibilities:
🏥 Patient Intake & Discharge
• Coordinate patient admissions and intake processes
• Support patient discharge arrangements and follow-up activities
• Ensure required information and documentation are completed accurately
• Coordinate with relevant teams to ensure a smooth patient transition
📦 Logistics & Patient Support
• Coordinate logistics required for patients receiving care at home
• Support arrangements for delivery of medications and other items to patients' homes
• Coordinate courier and delivery arrangements where required
• Track and follow up on delivery requirements to ensure timely service
🤝 Stakeholder Coordination
• Liaise closely with doctors, nurses and other healthcare stakeholders
• Coordinate information and operational requirements across different teams
• Respond to queries and follow up on outstanding matters promptly
• Support smooth day-to-day operations of the programme
📋 Programme Operations
• Maintain accurate records and operational documentation
• Coordinate daily schedules and patient-related activities
• Monitor ongoing operational requirements and escalate issues where necessary
• Provide administrative and coordination support to the programme team
• Perform other duties required to support programme operations
