Healthy Families America Program Coordinator – Help Families Build Stronger Futures

We have an opportunity where you can make an impact at both the family and program level.


As the Healthy Families America (HFA) Program Coordinator at Home Healthcare, Hospice and Community Services, you’ll help coordinate our Healthy Families America home visiting program within the Family Resource Center. You’ll support program fidelity, accreditation readiness, quality improvement, staff development, data and reporting, and community engagement while also maintaining an assigned caseload of families.


This role is ideal for someone who brings a strengths-based, trauma-informed approach to working with families and enjoys building collaborative relationships with staff, community partners, and other stakeholders.

In this position, you’ll help ensure families receive consistent, family-centered services while supporting the systems, staff, partnerships, and quality standards that make the program successful.


What You'll Do

  • Coordinate the Healthy Families America program and support fidelity to HFA Best Practice Standards.
  • Help develop and monitor program goals, objectives, performance outcomes, policies, and procedures.
  • Support accreditation readiness, quality assurance, continuous quality improvement, case record reviews, and program audits.
  • Assist with recruiting, hiring, training, supervision, coaching, and professional development of program staff.
  • Promote reflective supervision practices and help monitor caseload assignments, staffing patterns, and workload distribution.
  • Maintain an assigned caseload and conduct home visits according to HFA model requirements.
  • Partner with families to develop individualized Family Goal Plans and support progress toward identified goals.
  • Provide education and support related to parenting, child development, child safety, prenatal and maternal health, family wellness, and self-sufficiency.
  • Complete required screenings and assessments and connect families with healthcare, mental health, housing, employment, education, and social service resources.
  • Develop Family Resource Center activities that support HFA program goals.
  • Oversee program data collection, accuracy, documentation, and reporting and use data to support planning and improvement.
  • Build collaborative partnerships with healthcare providers, hospitals, schools, social service agencies, and community organizations.
  • Represent HCS in local and state collaborative initiatives supporting children and families.
