Home Based Residential Support Manager

Hours: 40 hours, 9 - 5, Monday- Friday. Hours are flexible as needed to accommodate visiting families schedules.
Summary
The Director of Home-Based Residential Supports will lead a multidisciplinary team to support multiple families and their individual needs. The individual in this role will actively partner with families to understand the individual’s support needs to maximize the well being of their loved ones.
Essential Job Functions
- Work with the director to effectively recruit, hire, train, assign, direct, and counsel HBRS staff.
- Responsible for evaluating incoming referrals to understand the needs of each individual case.
- Act as a liaison with families referred to the program to address concerns and goals while maintaining constant communication with HRBS staff.
- Coordinate care with outside resources such as OT, PT, Speech and any other service the client may need.
- Work collaboratively with the director of family and community services on planning implementing and managing the program in order to help families to deal with difficulties and overcoming challenges.
- Work collaboratively with the director of family and community services to ensure that all programs operate with quality and integrity. This will include but is not limited to ensuring programmatic guidelines and regulations are followed, ensuring compliance with DDS, CARF, and Incompass Human Services Policies and Procedures, and regularly reviewing and updating program specific policies and procedures. New policies and procedures are to be created on an as needed basis.
- Work collaboratively with director of family and community services, finance, and marketing teams to foster capacity for growth
- Work collaboratively with other programs across the agency to ensure individuals who receive services from multiple departments receive comprehensive services using a team approach and to ensure that individuals enrolled can become knowledgeable around community services and access them if there is a need.
- Work with the clinical department on an as needed basis to see that individuals served receive consult if needed as well as staff working in the department.
- Provide coverage in the event of extended staff leave and vacancies
- Act in a manner consistent with organizational values, mission and goals. Familiarizes self with and abides by personnel policies
- Serve as Mandated Reporter
- Position requires some on-call, evening or weekend work.
- This position requires some local travel.
- Other duties may be assigned as needed.
