At Vicar's Landing, our Home Health Aides play a vital role in helping residents live safely, comfortably, and independently. This position provides compassionate, person-centered care both within our community and in residents' homes, supporting their physical, emotional, and social well-being. Working under the direction of nursing leadership, you'll assist with daily living activities, monitor resident well-being, and help create meaningful experiences that enhance quality of life. What You'll Do

• Provide routine nursing care and assistance in accordance with residents' individualized care plans.
• Assist with resident admissions, transfers, and discharges.
• Support activities of daily living while promoting residents' comfort, safety, and independence.
• Observe, document, and communicate changes in residents' condition to the nursing team.
• Complete required documentation accurately in electronic medical records and daily flow sheets.
• Assist with meal service and other resident support activities as needed.
• Maintain a clean, safe, and sanitary environment consistent with healthcare standards.
• Participate in training, in-services, and team meetings to support continuous learning and quality care.
