Position Overview:

The Home Health Aide is a member of the Courtyards team who assists in meeting the personal care, social, and environmental needs of the Courtyard residents.

Essential Duties & Responsibilities:

• Appropriately assist clients with personal care activities in accordance with the personal services care plan including: Baths (tub, shower, bed bath), mouth-care, denture-care, skin-care, foot-care, ambulation, bathroom privileges (bedpan, commode, toilet), transferring, dressing & undressing activities, grooming activities, and positioning.
• Appropriately assist with household tasks directly essential to the client's health and in accordance with the plan of care, including: planning and preparing nutritional meals, assisting in feeding client if appropriate, light housekeeping of client's immediate environment, change linen, make bed, laundry of client's clothing & bedding.
• Count, dispense, and assist residents with medications; complete necessary documentation. Immediately report any changes in resident’s condition, incidents, or accidents, to the Charge person.
• Accurately measure and record oral, rectal, auxiliary temperatures, pulse, respiration and blood pressure when ordered.
• Monitors residents, recognizing and reporting any changes in resident condition to the supervisor.

Basic Duties & Responsibilities:

• Assist with medications, may not administer, may remind client, read label for client, bring medications to client, position client. Client must be self-directing.
• Assists in the orientation and training of new employees.
• Maintains cordial relationship with clients and their families.
• All other duties as assigned.
