What You'll Do
As a Home Health Physical Therapist, you'll help patients regain strength, mobility, balance, and independence while making a meaningful impact in their lives.
Responsibilities include:
- Perform comprehensive physical therapy evaluations and reassessments
- Develop individualized treatment plans based on patient goals and physician orders
- Provide evidence-based therapeutic interventions to improve mobility, strength, balance, endurance, and function
- Complete Start of Care assessments and ongoing therapy visits
- Assess patients' home environments and recommend equipment or safety modifications
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Collaborate closely with physicians, nurses, occupational therapists, speech therapists, medical social workers, and home health aides
- Complete timely and accurate electronic documentation
- Help patients safely recover while reducing hospitalizations and maximizing independence

Qualifications
Required
- Graduate of an accredited Physical Therapy program
- Current Arizona Physical Therapist license
- Current CPR Certification
- Valid Arizona driver's license
- Reliable transportation and current auto insurance
Preferred
- Home Health experience
- OASIS experience
- Strong communication and organizational skills
