Responsibilities:

• Start of Care (SOC) assessments
• Supervisory & skilled nursing visits
• Wound care & wound vac management
• Tracheostomy care
• Patient/family education
• Care coordination & EMR documentation

Requirements:

• Active Michigan RN license
• 1+ year nursing experience, preferred
• Valid driver's license & reliable transportation
• Strong assessment and communication skills

Home health, wound vac, trach, SOC, or supervisory experience preferred.
