Responsibilities:
- Completing the nursing process in the delivery of patient care, which includes assessment, planning, implementation, and evaluation of patient outcomes.
- Total oversight of patient from admission to discharge.
- Developing a plan of care based on findings.
- Serving as patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
- Timely completion of all skilled nursing documentation.
- Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
- Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff)
