DirectMed Home Health Services is actively seeking bilingual Polish-speaking Registered Nurses (RNs) to join our growing home health team and provide skilled nursing care and case management to patients throughout Chicago and the surrounding suburbs.

Our clinical services include the management of patients with:
- Diabetes
- Congestive Heart Failure (CHF)
- Hypertension (HTN)
- End-Stage Renal Disease (ESRD)
- Other chronic and complex medical conditions
DirectMed also specializes in caring for patients receiving outpatient dialysis, providing specialized clinical training and support to our nursing staff.
Competitive Per-Visit Compensation
We offer the following visit rates:

$100 – Start of Care (SOC)

$70 – Regular Skilled Nursing Visit

$75 – Recertification

$75 – Resumption of Care

$75 – Discharge Visit
Additional premium rates may be available for weekend visits.
Qualifications & Requirements
- Active Registered Nurse (RN) license in the State of Illinois.

Bilingual in Polish and English.
- Minimum of one year of nursing experience in a hospital, nursing home, or home health setting.
- Home health and OASIS experience preferred but not required.
- Excellent written and verbal communication skills.
- Ability to work independently and manage assigned patients.
- Reliable transportation.
- Strong organizational and time-management skills.
- A positive, dependable, can-do attitude.
- LPN applicants may also be considered for appropriate nursing opportunities.
Job Responsibilities
- Provide skilled nursing visits to patients in their homes.
- Case manage assigned patients and coordinate their care with physicians and other healthcare professionals.
- Conduct comprehensive nursing assessments.
- Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
- Develop and update individualized patient plans of care.
- Provide medication management, wound care, chronic disease education, and other skilled nursing services.
- Communicate with patients and families in Polish when appropriate.
- Coordinate visit schedules directly with assigned patients.

Complete all visit documentation within 48 business hours of the home visit.
- Maintain professional communication with DirectMed's clinical and administrative teams.
