Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures in home settings
- Monitor and evaluate patient health status, documenting changes and progress accurately
- Coordinate with physicians, therapists, and care team members to ensure continuity of care
- Educate patients and family members on disease management, medications, and home safety
- Maintain accurate clinical documentation in compliance with state and federal regulations
- Respond to patient needs and concerns with professionalism and empathy
Requirements:
- Active Registered Nurse (RN) license in the state of Maryland
- Current CPR/BLS certification required
- Minimum 1-2 years of nursing experience; home health experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for home visits
- Compassionate, self-motivated, and able to work independently
