Be the First Person a Family Trusts
The admission visit is the moment a family stops feeling alone. Done well, it changes everything that follows. We are looking for an experienced RN who can walk into a hard room, explain hospice with clarity and compassion, and give families confidence in the care ahead.
What You'll Do
- Perform hospice consultations and admissions, assessing eligibility and appropriateness of care
- Educate patients and families on hospice services and patient rights, with patience and honesty
- Communicate with physicians to secure admission orders, medications, and recertifications
- Participate in the Interdisciplinary Team Conference and plan of care review every 14 days
- Support documentation review to ensure patients continue to meet hospice qualifying criteria
- Provide clinical supervision, competency evaluations, and orientation support for nursing staff and CNAs
- Contribute to our QAPI program and to in-service education for staff and community partners
- Carry a part-time case load as assigned
What We're Looking For
- Active RN license in Idaho
- Two or more years of nursing experience, with hospice or home health experience strongly preferred. If you have worked in hospice, home health, palliative care, oncology, or long-term care, we want to hear from you.
- Expertise in pain and symptom management and end-of-life care
- Working knowledge of Medicare, Medicaid, state, and accreditation requirements
- Strong clinical documentation and communication skills
- Valid driver's license, vehicle insurance, and reliable transportation
- Schedule flexibility, local travel, and participation in the on-call rotation
