Duties/Responsibilities:

Provides patient care, ensuring patient safety and comfort, according to the Home Health Aide assignment form.

Reports and records any changes in condition or behavior of the patient.

Adapts to changes in workload or patient assignments without interference to patient care.

Responsible for completion of assignments, maintaining quality work.

Establishes good rapport with patients and families, respecting patients rights to privacy and confidentiality of information.

Accurately communicates in writing through utilization of appropriate charting methods, and turns in all paperwork on time.

Provides support to patients and families in a courteous manner.

Performs routine duties without physical supervision.

Accurately communicates patients information to the RN Case Manager, LPN and/or Assistant Director of Clinical Services.

Assists in maintaining equipment assigned to patient care, and reports malfunctioning equipment to appropriate personnel.

Any and all other duties and responsibilities as assigned.
