What You'll Do

As a Hospice LPN, you will play an essential role in providing compassionate, patient-centered care by:

• Delivering direct nursing care in accordance with state nursing practice standards
• Implementing and managing individualized plans of care based on comprehensive patient assessments
• Documenting patient care accurately and timely
• Educating patients and caregivers about diagnoses, treatments, and care plans
• Collaborating with the interdisciplinary hospice team to coordinate care and address patient needs
• Monitoring symptoms and providing effective pain and symptom management
• Evaluating patient responses to medications and treatments to ensure optimal comfort and quality of life

What We're Looking For

• Graduate of an accredited Practical Nursing or Vocational Nursing program
• Current LPN/LVN license in the state of practice
• Minimum of three years of nursing experience
• Hospice, home health, community health, or medical-surgical experience preferred
• Current CPR certification
• Strong communication, documentation, and organizational skills
• Valid driver's license and insured, reliable transportation
• A genuine understanding of and commitment to hospice philosophy and compassionate end-of-life care
