Location: Central East
Schedule: Tuesday - Saturday
Key Responsibilities
Complete patient assessments & update care plans
Provide symptom & pain management
Educate patients/families on care and medications
Coordinate with physicians and interdisciplinary team
Document timely, accurate clinical notes
Manage assigned caseload; ensure safe care environment
Participate in on‑call rotation
Requirements
Must have:
Current RN license
NLN‑accredited nursing program graduate
CPR certification
Valid driver’s license & insured vehicle
1+ recent year of RN experience or BSN
Ability to lift/position patients; frequent standing/walking
Preferred:
2+ years RN experience
Hospice/home health/public health experience

This position is based on an hourly visit compensation model.
Hourly ranges from $45 to $50 visit commensurate with experience and depending on type of visit.
