Housing Case Manager
Position Overview:
The DHS Housing Case Manager is responsible for providing comprehensive case management services to individuals and families experiencing homelessness or housing instability and are residing in a shelter setting. The Housing Case Manager works collaboratively with clients, service providers, landlords, and community agencies to develop individualized housing plans, connect clients to resources, promote housing stability, and support long-term self-sufficiency. The Housing Case Manager will work closely with the social service team.
Position Qualifications:
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field required. Minimum of two years’ experience providing case management or housing services. Experience working with individuals experiencing homelessness, mental illness, substance use disorders, or other vulnerable populations preferred. Knowledge of DHS housing programs, public assistance benefits, and community resources. Strong organizational, communication, and documentation skills. Ability to manage multiple cases while meeting deadlines.
