This is a non-exempt, full-time position with benefits.
This position integrates intake, case management, housing navigation, and payment processing to support homeless individuals with intellectual and developmental disabilities (IDD) in achieving housing stability. This role serves as a central point of contact for clients, partnering with them to provide assessments, referrals, and direct assistance. Responsibilities include processing payments on behalf of clients, guiding them through resource navigation, and delivering trauma-informed, person-centered case management services that center dignity, choice, and autonomy.
Essential Functions:
- Practices solidarity with the disability community by promoting self-advocacy, respecting informed choice, challenging ableist barriers within systems, and amplifying client voice in housing and service planning decisions.
- Provides housing-focused stabilization case management to help clients secure permanent housing and maintain housing stability.
- Partners with clients on goal setting and acquiring vital documents. Ensures clients remain responsive to client-identified goals and priorities throughout program participation.
- Supports clients in navigating and self-advocating through disability determinations and in enrolling in the Medicaid SLS waiver and Home and Community-Based Services.
- Works with IDD Housing Navigator to help clients access affordable housing options, mediating with landlords, host home providers, and residential agencies, and applying for housing voucher programs.
- Connects clients with long-term services and support, including food access, transportation, benefits counseling, employment, recreational, and health services.
- Provides crisis intervention case management that addresses basic needs and leads to stabilization.
- Develops, implements, monitors, and modifies service plans with clients.
- Develops exit plans and guides clients through a successful transition from the program.
- Collaborates with other case managers, doctors, organizations, caregivers, family members, etc. to develop creative solutions to complex problems for participants.
- Maintains accurate case records in required CRMs; documents client contacts and progress to ensure accountability.
- Allows new staff and interns to shadow and sit in on client appointments.
- Assists with reporting as needed by providing client testimonies.
- Collaborates with community partner agencies.
- Other duties as assigned.
