Primary Work Location: Multisite

Multisite staff work across several program locations throughout Alameda County, providing care and support wherever it is most needed. These sites offer a range of services, including post-hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support

Job Summary:

The housing coordinator will assist clinical staff and shelter residents in the coordination of medical and behavioral health care and works alongside a diverse clinical team at a medically supported shelter program serving people experiencing homelessness who are recovering from recent hospitalization, injury, or illness. The housing coordinator will provide a variety of functions to support Cardea Health clinical programs at the Medically Supported Shelter. The ideal candidate is organized and motivated. The housing coordinator supports resource linkage and develops housing plans for individuals who have experienced chronic homelessness. The housing coordinator helps individuals navigate the complex housing system, identify the appropriate resources they will qualify for, and help them successfully exit the medically supported shelter program in a timely manner. The ideal candidate has experience with homeless or incarcerated populations. This is an excellent position for someone who is seeking to gain experience in a healthcare social service setting.
