Housing Coordinator
The Housing Coordinator (case manager) provides comprehensive person-centered case management services to individuals and families experiencing housing instability for those who are enrolled in Medicaid-funded programs. The Case Manager will assist clients in securing and maintaining safe, affordable housing while connecting them to community-based resources and supportive services. This role integrates housing placement, tenancy support, and healthcare coordination to promote long-term housing stability and reduced hospital utilization.
HC will manage a caseload of 20 to 21 housing clients on Staten Island.
Key Responsibilities
- Conduct intake assessments and develop individualized service plans
- Conduct comprehensive biopsychosocial and housing assessments
- Assist clients with landlord/property managers coordination
- Assist clients with housing applications (NYCHA, supportive housing, rental subsidies, CityFHEPS, etc.)
- Provide crisis intervention and ongoing case management support and eviction prevention support
- Connect clients to employment services, public benefits, mental health services, and other community resources
- Maintain accurate and timely case notes and documentation
- Monitor client progress and adjust service plans as needed
- Participate in case conferences and team meetings
- Support clients with lease compliance, budgeting, and independent living skills
- Collaborate with medical and behavioral health providers under Health Home guidelines
- Assist with Medicaid benefits and entitlement recertification
- Ensure compliance with MRT standards, and housing program regulations
- Participate in case conferences and quality improvement initiatives
