Housing Navigator (52245)
Summary
Provide street outreach and navigation to help homeless individuals in Fresno/Madera secure permanent housing via case management and referrals.
POSITION SUMMARY:
Person in this position will be responsible for street outreach and navigation to identify individuals and those with families who are homeless or at imminent risk of becoming homeless within Fresno and Madera County’s. The Navigator will provide individualized client support throughout the process by helping clients through the Coordinated Entry System (CES) and address any barriers to obtaining permanent housing. Essential Job Functions include those listed below.
ESSENTIAL JOB FUNCTIONS:
- Have a strong working knowledge of housing services, supportive services, and community resources.
- Conduct outreach activities throughout Fresno-Madera County including street outreach, engagement, assessment, and navigation.
- Intake and assessment of incoming participants to determine eligibility and appropriate services.
- Manage of a caseload of participants including maintenance of client files, development of service plans, and tracking pertinent data.
- Regular contact with members to ensure there are no gaps in the activities designed to address an individual’s health and social service needs, and swiftly addressing those gaps to ensure progress towards regaining health and function continues.
- Link individuals to needed programs and provide warm handoffs.
- Assist individuals in securing documentation and services needed to obtain housing.
- Assist individuals in applying for housing vacancies, completion of supportive and subsidized housing paperwork, and monitoring progress to completion.
- Receive referrals from Cal-Aim Enhanced Care Management (ECM).
- Make and receive referrals to Cal-Aim Community Supports (CM) housing services for deposit assistance.
- Ensure clients transition smoothly to permanent housing.
- Follow clients through and among services to achieve housing stability and retention.
- Navigate clients until they are linked to appropriate housing intervention or services are no longer needed.
- Heavy documentation; WestCare’s Clinical Data System.
- Build and maintain relationships with property managers and property owners.
- Attend required Fresno Madera Continuum of Care (FMCoC) CES and Navigator trainings.
- Provide weekly updates to Master by Name List and participate in weekly FMCoC case conferencing meetings.
- Provide all services in linguistically and culturally appropriate manner, including use of interpreters for those with limited English proficiency.
- Transportation of clients in company vehicle.

- Perform all other duties as assigned.
