What You'll Do

As a Housing Placement Case Manager, you'll provide comprehensive case management services to families transitioning from our Family Center into stable, affordable housing.

In This Role, You Will:

  • Build trusting relationships with families through strength-based and trauma-informed case management.
  • Conduct intake assessments, establish goals, develop housing plans, and monitor progress toward successful housing placement.
  • Help families identify and overcome barriers that impact housing stability and self-sufficiency.
  • Connect residents with critical community resources, social services, and support programs.
  • Educate residents on available services and assist with applications and required documentation.
  • Maintain accurate case notes, reports, and resident records in accordance with program requirements.
  • Ensure timely and accurate data entry within the Homeless Management Information System (HMIS).
  • Meet regularly with residents to evaluate progress, provide encouragement, and adjust housing plans as needed.
  • Facilitate financial literacy education and budgeting strategies that support long-term housing success.
  • Advocate on behalf of residents with landlords, community agencies, service providers, and housing partners.
  • Collaborate with KidZone early education staff and other service providers to support the entire family's well-being.
  • Participate in case consultations and interdisciplinary meetings to identify solutions and share resources.
  • Conduct monthly aftercare visits and follow-up support to help families maintain housing stability and prevent future homelessness.
  • Represent Samaritas at community events, outreach activities, resource fairs, and networking opportunities.

We're Looking For Someone Who Has:

  • Strong communication and interpersonal skills.
  • Excellent organizational and documentation abilities.
  • The ability to work independently while managing multiple priorities.
  • A collaborative approach to helping individuals achieve meaningful goals.
  • Knowledge of community resources and housing-related services.
  • Compassion, patience, and cultural sensitivity when supporting families experiencing crisis.
  • Experience using Microsoft Office and database systems, including HMIS or similar software.

Education & Qualifications

Required

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field.

Preferred

  • One or more years of experience in human services, community services, or case management.
  • Experience working with families, adults, or individuals experiencing crisis, homelessness, or housing instability.

Additional Requirements

  • Valid driver's license with a good driving record.
  • Reliable personal transportation.
  • Proof of current automobile insurance.
  • Ability to travel locally to resident homes and community partner locations as needed.
