Position Overview
This is a fixed term position that will end on 6/30/2028.
The HealthierHere Community Hub is a community-centered entity that centralizes referrals from across King County to connect people to health-related social needs services and benefits. The Hub brings together community-based, behavioral health, and healthcare organizations to provide culturally responsive care coordination support for community members. The Hub’s benefits and services are free for everyone.
The HRSN Eligibility and Referral Specialist- Home Modifications will initially report directly to the Director of Program Operations and is responsible for supporting the HRSN Screening and Referral contract that HealthierHere has with the City of Seattle Aging & Disability Services.
The HRSN Eligibility and Referral Specialist will confirm client eligibility for contracted Health Related Social Needs (HRSN) services, perform HRSN assessments with clients over the phone, enter data in two client Management systems (GetCare and Connect2 Coordinator, the HealthierHere Community Hub’s case management platform), authorize and enroll clients in services, refer clients to direct service providers, and manage the process of creating, printing and mailing approval and service denial letters. The HRSN Eligibility & Referral Specialist will also have the responsibility of managing the competitive bidding process to source appropriate vendors for Home Accessibility Modifications and Remediations. The HRSN Eligibility and Referral Specialist will need to develop strong familiarity with the HealthierHere Community Hub and HRSN program technology, functions, workflows, and community resources to be able to optimally support the program and its partners.
The ideal candidate brings experience in health or community services and has provided care coordination, health navigation, and client assessments directly to underserved individuals, families, and communities. They understand home modifications and adaptations that help seniors and people with disabilities remain safely in their homes and build authentic, respectful relationships with people from diverse backgrounds.
