This unique hybrid position allows you to provide person-centered care in both our Intermediate Care Facility (ICF) and Blue Ash Day Program, helping individuals thrive in supportive residential and community settings.

Position Schedule & Locations

Monday, Tuesday & Wednesday

Intermediate Care Facility (ICF)

• 6:00 AM – 9:30 AM
• 3:00 PM – 6:00 PM

Blue Ash Day Program

• 9:30 AM – 3:00 PM

In this hybrid role, you will provide nursing oversight, medication administration, health monitoring, and individualized support while collaborating with interdisciplinary teams to ensure high-quality care.

✅ What You Need

• Current Ohio RN or LPN license
• Strong clinical assessment and communication skills
• Ability to work independently across multiple service locations
• Trach/vent experience preferred (not required, we'll train you)
• A genuine passion for compassionate, person-centered care
