Responsibilities

• Provide compassionate care and assistance with daily living activities.
• Monitor and document clients' health status and report any changes.
• Assist with medication management and reminders.
• Support clients with mobility and physical exercises.
• Engage clients in social activities to promote mental well-being.
• Communicate effectively with family members regarding client needs.
• Maintain a clean and safe living environment for clients.
• Uphold confidentiality and respect client dignity at all times.

Qualifications

• Proven experience in geriatrics or senior care.
• Knowledge of post-acute care practices.
• Familiarity with sub-acute care settings.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Basic first aid and CPR certification.
• Experience with medical-surgical environments.
• Willingness to undergo background checks as required.
