The Intake Coordinator assists individuals while admitting to Key Works' Long-term Substance Use Disorder Treatment program. This position is responsible for providing exceptional customer service while admitting individuals into residential care, orienting, and assisting patients in complying with program and treatment requirements consistent with all clinical policies and procedures. The Intake Coordinator completes all admission paperwork documentation in the electronic medical record and completes all aspects of checking the patient into the program. When time permits, the Intake Coordinator helps patients prepare for their discharge from Key Works.
Primary Responsibilities:
Admit patients and provide support to their families during the intake process. These tasks include but are not limited to:
- Completing patient intake paperwork.
- Greet new patients and new patient families.
- Help orient and familiarize patients with Key Works treatment protocols.
- Connect the new patient to their identified peer support.
- Work with appropriate staff to determine room assignments for new patients, review program guidelines, give facility tour and review medical emergency sheets within 12-hours of check in.
- Log all patient belongings and obtain patient’s signature. Place patient belongings in the appropriate storage space and determine what a patient can keep with them while in treatment.
- Record patients’ external appointments (medical, legal, etc.) in the electronic medical record so it is updated on the daily run/transportation list
- Collaborate with the transportation department to make sure new admissions have rides to and from appointments
- Collaborate with the Medication Room Coordinator when patients bring medications in at the time of admission
- Collaborate with Medication Room Coordinator and case management to ensure patient has continuity of care with medication providers
- Update the communication log with information regarding the new admission
- Transport patients if necessary – only use Key Works vehicles
- Model appropriate behavior, communication, and dress code according to Key Works policies
- Complete all incident reports as required Key Works procedures and policies
- Apply knowledge of organization’s policies and procedures to uphold these policies with patients and residential patient family members.
