Intake Manager

JOB SUMMARY

The Intake Manager communicates and coordinates with patients, families, community liaisons, and referral sources. Process referrals and coordinate patient care. Assist to ensure smooth back office operations. Helps ensure adherence to regulations, policies, and procedures. They are responsible for back-office personnel, directly, and the delegation to and coordination of intake & scheduling personnel evaluations; is responsible for maintaining standards of operation to comply with federal and state regulations and guidelines and Agency specific goals.

DUTIES & RESPONSIBILITIES

Plans, organizes and directs staff assignments and staff performance to ensure daily referral and intake tasks are completed timely and accurately.

Assists with performance management and development, and disciplinary actions for intake + scheduling staff, with support from Operations Director as needed.

Maintains ongoing, positive work relationships with all agency stakeholders and members.

Prepares or assists in staff meetings and provides training to all intake staff concerning pertinent changes, process improvements or implementation of feedback.

Is responsible for oversight and implementation of new hire training for intake staff.

Respond to incoming phone calls to handle referrals, scheduling, and coordination of details to support field staff, patients, families, and referral sources.

Assist field staff by gathering and verifying information, coordinating schedules, and supporting workflow processes.

Responsible for Homecare Homebase workflow appropriate to the Intake, Scheduling, or Back-office team roles.

Responsible for pulling and reviewing intake reporting related to daily, weekly or monthly admissions goals, scheduling workflow, and daily performance.

Work with agency leadership to provide other types of office support as needs dictate.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

2-5 years of experience, preferably in intake/referrals in a home health environment preferred.

In-Person office setting Monday – Friday with availability to support on weekends as needed.

Strong written and oral communication skills in person and over the phone.

Excellent customer service skills

Strong team leadership skills

Organized

Flexible—responds well to changing priorities and conditions
