Emerest Health of PA is seeking a dedicated and compassionate Intake Specialist to join our team and serve clients throughout Venango County and the surrounding areas. This position plays a vital role in ensuring clients receive high-quality care and support from the very beginning of their service journey.
Qualifications:
- Minimum of 2 years of experience in home care, healthcare, or a client-focused setting
- Must reside in Pennsylvania
- Willingness to travel throughout the assigned coverage area
- Ability to work occasional evenings and weekends as needed
- This is not a remote position.
Responsibilities:
- Conduct initial client intakes and comprehensive home assessments with clients and their families
- Perform quality assurance and supervisory visits in clients' homes
- Assess the quality of care being provided and ensure compliance with agency policies and standards
- Document and report findings from quality assurance and supervisory visits
- Collaborate with clients, families, and care teams to support positive client outcomes
