Intensive Case Manager- Day Shift

New Horizons is a nonprofit agency seeking a caring and compassionate Intensive Case Manager to provide medically necessary quality intensive case management services to individuals with severe and persistent mental illness. As an Intensive Case Manager your responsibilities will include:
- Engaging in staff hours of service for assigned individuals at the rate of 115 hours per month.
- Responsible for direct and indirect services to maximize revenue generated by NHTC within the bounds of ethical principles and good business practice.
- Responsible for completing TCM assessments within the first thirty day of new admission.
- Monitors client progress toward treatment goals in all service modalities in which the individual is enrolled.
- Responsible for coordinating individuals care both within and outside NHTC
- Prepares written documentation for each service conducted
- Responsible for developing written service plans developed with participation from the individual, family and or other professionals.
- Responsible for monitoring client progress towards treatment goals in all service modalities.
- Coordinates referrals, admissions, discharges and transfers between service modalities.
- Assist individuals with obtaining benefits such as social security, medical insurance, food stamps, housing subsidies and monitors individual's financial status.
- Provides timely and effective crisis intervention in accordance with the program's standards and protocol for conducting this activity.
- Provides On-call emergency service as schedule for after business hours seven day a week.
- Provides transportation as needed while adhering to agency transportation police and procedures.
- Miscellaneous duties according to instruction.
