Licensed Practical Nurse

AlphaCare Home Health Agency is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our home health team. In this role, you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse while helping patients maintain comfort, independence, and quality of life.
We are looking for a dedicated clinician who works well independently, communicates effectively with patients and care teams, and is passionate about delivering quality patient-centered care.
Responsibilities Include:
- Provide skilled nursing care in accordance with physician orders and care plans
- Monitor patient conditions and report changes to the supervising Registered Nurse
- Assist with implementing and updating individualized plans of care
- Administer treatments and medications as ordered
- Educate patients and caregivers on treatments, medications, and home care needs
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary team members to support continuity of care
- Recognize and report emergency or significant clinical situations appropriately
- Participate in weekend and holiday coverage as needed
Requirements
- Current Massachusetts LPN license
- Graduate of an accredited Practical Nursing program
- Minimum one (1) year of nursing experience required
- Home Health experience preferred
- Current BLS certification
- Strong communication, organizational, and clinical skills
- Ability to travel throughout assigned service areas
