Description

Home Health Licensed Practical Nurse (LPN)

Community Health Network is seeking a dependable, organized, and compassionate Licensed Practical Nurse (LPN) to join our home health team.

If you are an LPN who values schedule flexibility, independence, variety in your workday, meaningful patient relationships, and a supportive team environment, this may be the right opportunity for you.

Our LPNs serve a diverse home health population, including individuals with intellectual and developmental disabilities (ID/DD), individuals residing in group homes, patients living independently in the community, and patients requiring traditional skilled home health nursing visits.

No two days are exactly the same. Depending on your assigned schedule and caseload, your day may include traveling to a group home for a nursing visit or medication pass, seeing an individual living independently for skilled nursing care, or providing ongoing assessment, education, medication management, and treatments in a patient's home.

  • Option to work three 12-hour shifts or four 10-hour shifts

  • Independent field-based work with support from our clinical and administrative team

  • Variety of patients, settings, and nursing needs

What You’ll Do

  • Provide skilled nursing care for patients with a variety of diagnoses and healthcare needs

  • Work with individuals with intellectual and developmental disabilities (ID/DD) and other complex support needs

  • Complete nursing observations and assessments within the LPN scope of practice

  • Provide scheduled medication administration and medication passes in group home settings

  • Assist with medication management and monitor for medication effectiveness, side effects, and potential concerns

  • Administer ordered medications and treatments

  • Perform skilled nursing procedures and treatments within the LPN scope of practice

  • Monitor patients for changes in condition and promptly communicate concerns to the RN Case Manager, physician, and other appropriate members of the care team

  • Provide disease-process, medication, and health-management education to patients, families, caregivers, and residential staff

  • Reinforce established plans of care and patient-specific interventions

  • Promote patient safety, independence, and quality of life within the home and community

  • Collaborate with RN Case Managers, physicians, therapists, caregivers, residential providers, direct-support professionals, and other members of the interdisciplinary team

  • Maintain accurate, timely, and compliant clinical documentation

  • Participate in care coordination and interdisciplinary communication as needed

What We’re Looking For

  • Current Licensed Practical Nurse (LPN) license in the State of Ohio

  • Current CPR/BLS certification

  • Valid driver's license, reliable transportation, and automobile insurance

  • Comfortable traveling between patients in private homes, group homes, and community settings

  • Strong nursing observation, clinical judgment, and communication skills

  • Excellent organization and time-management skills

  • Ability to recognize and promptly report changes in patient condition

  • Ability to work independently in the field while collaborating effectively with the RN and interdisciplinary care team

  • Comfortable utilizing electronic medical records and completing clinical documentation

  • Home health, ID/DD, group home, medication administration, long-term care, skilled nursing, or community nursing experience is a plus

  • New to home health or the ID/DD population? We are willing to train the right LPN.
