Summit Home Care and Hospice is searching for an experienced and motivated Licensed Practical Nurse (LPN) to join our Home Health team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Full-Time Licensed Practical Nurse (LPN):

• Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
• Pay Rate: $28.00 - $31.00 per Hour
• Location: Grove City, OH & Surrounding Areas
• Expected Travel: ~30 Mile Radius (Mileage Reimbursement)
• Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assesses and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.
