The Care Team Hospice is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to support hospice patients at The Gables of Green in the Akron/Green, Ohio area.

This is a unique opportunity for an LPN who enjoys building meaningful relationships with patients, families, and facility staff while providing skilled nursing care focused on comfort, dignity, and quality of life.

Unlike a traditional field-based hospice position, you'll spend most of your time supporting patients at one primary facility, with occasional visits to patients within approximately a 20-mile radius as needed.

We're looking for an LPN who is dependable, clinically confident, comfortable working independently, and passionate about making a difference in patients' lives.

Schedule & Location

• Full-Time | Approximately 30–40 Hours Per Week

• Primary Location: The Gables of Green – Green, OH

• Limited local travel within approximately a 20-mile radius

• Primarily facility-based with additional patient visits as needed
