Responsibilities
- Provide direct nursing care to patients according to physician directives and care plans.
- Monitor patient health by taking vital signs and observing overall condition.
- Administer medications and injections as prescribed.
- Maintain accurate and detailed patient records and documentation.
- Collaborate with healthcare professionals and family members to develop and adjust care plans.
- Educate patients and families on treatment plans, care techniques, and health maintenance.
- Assist with basic patient care, including hygiene and mobility support as needed.
- Respond promptly to patient emergencies and communicate issues to supervisors.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
- Prior experience in home health care or a related nursing environment is a plus.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation to travel to patient homes within Westland, MI area.
- Basic computer skills for documentation and reporting.
