Licensed Vocational Nurse

We’re seeking a Licensed Vocational Nurse (LVN) to join our growing team and help us make a difference in the lives of the families we serve.

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse (RN) and according to the patient’s individualized plan of care.

• Administer medications, treatments, and wound care as prescribed.

• Monitor patient conditions and promptly report any changes to the supervising RN and physician.

• Educate patients and families on medications, treatments, and overall care plans.

• Maintain accurate clinical documentation and patient progress notes in compliance with agency policies and state/federal regulations.

• Collaborate with the healthcare team to ensure continuity and quality of care.

• Follow infection control, HIPAA, OSHA, and home health compliance standards.

Requirements

• Active Licensed Vocational Nurse (LVN) license in the state of Texas.

• Minimum 1 year of nursing experience preferred (home health experience is a plus).

• Strong clinical assessment and patient care skills.

• Ability to work independently and manage multiple patient cases.

• Reliable transportation, valid driver’s license, and current auto insurance.

• Must pass a background check.

Preferred Qualifications

• Prior experience in home health, hospice, or skilled nursing facilities.

• Knowledge of Medicare/Medicaid documentation and compliance.

• Familiarity with OSHA, HIPAA, and EVV (Electronic Visit Verification) requirements.
