Discussion

As a Social Worker Assistant, you’ll have the opportunity to make a meaningful difference in the daily lives of our residents by helping them feel heard, supported, connected, and valued. Working under the direction of a licensed Social Worker, you’ll provide hands-on support to residents and families while helping address their social, emotional, and everyday needs.

You’ll build genuine relationships with residents, assist with admissions and transitions, support social engagement and activities, help connect residents and families with resources, and serve as a caring presence when residents need encouragement or assistance. You’ll also work closely with the interdisciplinary team to help identify concerns and ensure important resident needs are communicated and addressed.

This is an ideal opportunity for someone who is passionate about helping others, building meaningful relationships, advocating for residents, and creating a sense of community. Your work may seem like small moments—a conversation, a helping hand, or simply being there—but those moments can make a lasting difference in a resident’s experience.

  • Develop one-to-one relationship with residents and family as needed for counseling and identifying needs.
  • Assist family and resident during admission process.

  • Work with family, community resources, and appropriate facility staff as needed to solve financial needs and promote emotional security.
  • Assist Administrator and Director of Social Services with resident/family/responsible party concern management.
  • Attend committee meetings as required.
  • Attend weekly Medicare meeting.
  • Assist with room changes by contacting families/resident and completing necessary forms.
  • Refer residents for vision, dental, and hearing services.
  • Office work, such as copying documents and filing.

Educational/Training Requirements

  • Must have graduated from high school. Preferred to have some college, preferably in Psychology or Social Work. Will consider current College students.

Experience Requirements

  • Previous experience in a skilled nursing facility is preferred.
