Licensure & Certification
Active Iowa Medical License
Active DEA (with Iowa address)
Active CDS
Board Certified in:
ABEM, ABFM, or ABIM (or osteopathic equivalents)
Must be residency trained in specialty certified in
ACLS, ATLS, and PALS required
If ABEM-certified: must provide current ACLS and one-time proof of ATLS
Additional Documentation Required

Case logs from past 2 years
Copies of all education & training diplomas
