Position Summary

The Licensed Practical Nurse (LPN) provides quality nursing care to residents, promotes their health and well-being, and helps ensure a safe, comfortable, and supportive environment. The LPN works collaboratively with nursing staff, physicians, residents, families, and other healthcare professionals to deliver person-centered care.

What You'll Do

• Assess residents' conditions, monitor changes in health, and communicate concerns promptly.

• Administer medications and treatments in accordance with physician orders and applicable regulations.

• Monitor and document vital signs, food and fluid intake, and other relevant clinical information.

• Assist with developing, implementing, and updating resident care plans.

• Communicate with physicians, residents, families, and the interdisciplinary care team regarding changes in condition and care needs.

• Provide guidance and support to nursing personnel and assist with staff training and orientation as needed.

• Maintain accurate clinical documentation, assessments, and incident reports.

• Promote resident safety and ensure compliance with infection control, workplace safety, and regulatory requirements.

• Participate in quality improvement initiatives, surveys, and ongoing education.

• Help create a respectful, compassionate environment where residents receive the highest quality of care.

What We're Looking For

• Current, active Licensed Practical Nurse (LPN) license in the state of Indiana.

• Graduation from an approved nursing program.

• At least one year of related nursing experience preferred.

• Strong communication, organization, and clinical assessment skills.

• A compassionate, resident-focused approach to care.

• Ability to work collaboratively with residents, families, and team members.
